Learn to Swim and membership forms can be downloaded below. Please note that enrolment and membership application forms are not available, as they will be provided at the time of registration. All enrolments and membership applications must be done in person at the centre.
Please click on the required form to download, then print, complete and return. Forms can be returned to reception when you next visit the centre or you can email them to email@example.com. For security reasons we cannot accept credit card or bank account details via email.

Updating Debit Details - If you need to update your bank account or credit card details for your Learn to Swim lessons or membership please see reception for a modification form when you next visit the Centre. For security reasons we cannot accept these details electronically.
Learn to Swim Forms
Learn to Swim Cancellation Form(PDF, 108KB) - A Cancellation Form must be submitted at least five days prior to the next debit date or the payment will be deducted in full. Once processed, a confirmation will be sent via email.
Learn to Swim Medical Suspension Form(PDF, 200KB) - If you have missed two to four consecutive lessons, please complete this form and return with a medical certificate covering the relevant dates.
Learn to Swim Missed Lesson Form(PDF, 149KB) - If you have missed a single lesson you may be eligible for a family pass. This is limited at one per student per term.
Learn to Swim Calendar 2017(PDF, 289KB) - This includes debit dates, term dates and public holidays.
These forms are for members with a direct debit membership.
Membership Cancellation Form(PDF, 92KB) - A Cancellation Form must be submitted at least five days prior to the next debit date or the payment will be deducted in full.
Membership Suspension Form(PDF, 284KB) - If you are going away for 1 week or more you have the option to suspend your membership. Please allow at least 5 days notice prior to the first day of suspension. Medical suspensions can be processed upon presentation of a valid medical certificate.
