Intake Information and Assessment Services
Stonnington Aged Services Intake, Information and Assessment Service provides assistance and information regarding access to the services provided by Aged Services and other support options. The Intake and Information Officer determines eligibility for access to these services, as well as provide information on other available community supports and options. If the applicant is eligible for services, the Intake and Information Officer will book a time for an Assessment Officer to visit and conduct an in home assessment.
All eligible people referred to Aged Services receive an in-home assessment, where a skilled Assessment Officer identifies client needs and priority for service. The assessment identifies the person's strengths, needs and abilities, and then aims to provide services or care. A care plan is developed in consultation with the client (and carer if applicable), and includes information on the types of services or supports to be provided, frequency of these services, and may include referrals to other appropriate community based support services.
Eligibility
The services provided by Aged Services are targeted towards frail older people, people with a disability, and the carers of both. Eligibility and access to services is subject to a number of factors. Services are allocated to eligible residents according to assessed and prioritised need.
