Immunisation Booking

 If you attend immunisation sessions at our Prahran or Malvern East venues, a pre-booking system exists for day sessions.

Venues are:                                                                                                                                                                                                             Princes Gardens Maternal and Child Health Centre (22 Little Chapel Street, Prahran - Melway Ref: 58 E5)                               Phoenix Park Children's Hub, Maternal and Child Health Centre (25 Quentin Road, Malvern East - Melway Ref: 69 E2)

Two nurses will be at each session and booking times will be in a block of six infants/families every 15 minutes.

If you would like your child to be immunised at one of these venues, please complete the Immunisation Booking Request Form below. Your request for a booking will be passed onto an officer who will contact you directly with details.

The officer will take your child’s details, make an appointment time and give you a reference number.                                              You will need to quote your reference number on the day.  An appointment is not confirmed unless a reference number has been advised.

Appointments can be made via telephoning the Council on 8290 1333.

All sessions already fully booked are not shown on the form.
