Membership forms can be downloaded below. Please note that membership applications are not available, as they will be provided at the time of registration. All membership applications must be done in person at the centre.
Please click on the required form to download, then print, complete and return. Forms can be returned to reception when you next visit the centre or you can scan and email them to email@example.com. For security reasons we cannot accept credit card or bank account details via email.

Updating Debit Details - If you need to update your bank account or credit card details for your membership please see reception for a modification form when you next visit the Centre. For security reasons we cannot accept these details electronically.
Membership Cancellation Form(DOC, 152KB) - A Cancellation Form must be submitted at least 5 days prior to the next debit date or the payment will be deducted in full.
Membership Suspension Form(DOC, 142KB) - If you are going away for 1 week or more you have the option to suspend your membership. Please allow at least 5 days notice prior to the first day of suspension. Medical suspensions can be processed upon presentation of a valid medical certificate.
