Community Matters: Planning for your future health care

Advance care planning promotes care that is consistent with a person's goals, values, beliefs and preferences. It prepares the person and others to plan for future health care, for a time when the person may no longer be able to communicate those decisions themselves. Join a spokesperson from Alfred Health Advance Care Planning Service to find out more.

In recognition of Dying to Know Day on Sunday 8 August, an annual day of action dedicated to activating conversations and community actions around death, dying and bereavement.

This event will be presented via Zoom. To join you will need an internet-enabled computer, tablet or smartphone. The video link will be emailed 2-3 days prior to the session.

How to use Zoom

On your computer

1. Click on the link in the event confirmation email you received after registering and you will be prompted to download and install Zoom.
2. Enter Meeting ID or link, and the Password in your event confirmation email.

On your tablet or smartphone
1. Download the Zoom Cloud Meetings app from your Apple App Store or Google Play Store prior to the workshop.
2. Enter Meeting ID or link, and the Password in the event confirmation email sent to you after registering.
